You may provide supporting material such as a narrative of the proposed project and a detailed budget. You are encouraged to submit the application electronically by April 1 to the committee chairperson, listed below. If you are unable to submit electronically, then send four copies of the completed and signed form to the committee chairperson in time to arrive by April 1.
